Below are the forms needed to sign up for our tournament. Please read everything on this page in it's entirety. Thank You.

  1. All of the forms required to sign up for our tournament are listed below.
  2. Click on the 2012 Team Registration link and print out the form.
  3. Page 2 of the Team Registration Form is for your records - there is no need to mail it in to us.
  4. Next you will need the Accident Waiver Forms for your players, manager and coaches.
  5. Click on the 2012 Players Waiver Form link.
  6. Print out the Player Waiver Form - 1 per player
  7. Please have the parent/guardian fill out the form in it's entirety - filling in each field marked with an "X".
  8. Click on the 2012 Manager/Coach Accident Waiver Form link.
  9. Print out one form for each manager, coach or acting coach on your team.
  10. Please be sure to fill out the forms in their entirety - filling in each field marked with an "X".
  11. Please include a copy of your team's insurance papers.
  12. When your Team Registration form is filled out, ALL of your Waivers are signed and your team's insurance papers are copied, mail those items and your check to the address listed below.
  13. Please be sure your forms are spelled correctly and are legible.

    Accident Waiver Forms

    ATTENTION: Please be sure all lines marked with an "X" are filled in completely and are legible.

    This form can not be altered in any way.

    The staff of the Sunshine Fastpitch Invitational understands that the process of getting these forms signed can get frustrating. These forms have to be signed for three reasons:

    1) We have to protect the foundation at all costs especially in today's society.

     2) We would not be able to use Sunshine's name in the title of the tournament or reference a logo.

    3) We cannot have the event without them. 

    We thank you in advance for your patience and efforts while going through this process. Once the paperwork is done, we can all focus on a great day of softball, sportsmanship and fundraising.

    1. Please print out a player waiver for each player on your team.

    2: Please have the parent/guardian fill in the players name at the top.

    3: Please have the parent/guardian sign, print & date the form.

    4: Please have a witness sign and mark down their address on the lower two lines.
