- All of the forms required to sign up for our tournament are listed below.
- Click on the animated 2010 Team Registration link and print out the form.
- Page 2 of the Team Registration Form is for your records - there is no need to mail it in to us.
- The next form you will need is the Accident Waiver Form.
- Click on the Accident Waiver & Release Of Liability Form link.
- Print out one form for every player on your team.
- The forms are to be signed by each player along with their parent or guardian.
- When your Team Registration form is filled out and ALL of your Waivers are signed, mail those forms and your check to the address listed below.
- Please be sure your forms are spelled correctly and are legible.
- You can visit the FAQ tab for the remaining requirements.
