  1. All of the forms required to sign up for our tournament are listed below.
  2. Click on the animated 2010 Team Registration link and print out the form.
  3. Page 2 of the Team Registration Form is for your records - there is no need to mail it in to us.
  4. The next form you will need is the Accident Waiver Form.
  5. Click on the Accident Waiver & Release Of Liability Form link.
  6. Print out one form for every player on your team.
  7. The forms are to be signed by each player along with their parent or guardian.
  8. When your Team Registration form is filled out and ALL of your Waivers are signed, mail those forms and your check to the address listed below.
  9. Please be sure your forms are spelled correctly and are legible.
  10. You can visit the FAQ tab for the remaining requirements.
