Please complete the form and send it to the address listed on the form. Please print clearly and check your spelling. Click on the Ad Book tab for examples on sizes, prices, etc. The team shoutout's may contain any wording of your choice and/or your team logo or mascot. We will do our best with sizing and detail. Please keep in mind that we are not professional printers but we will do our very best. Any ad's or logos will be sent back to you for final approval on size, lay out and overall appearance. If you should need an idea on an ad layout, please go to our ad book and view ad's from our teams and sponsors from 2008.
